I'm Dr. Michael Karlfield, and today we're going to shed light on a silent, often overlooked saboteur in chronic illness recovery. Mold. This episode, especially close to my heart because of how many patients I've seen struggle to get better from Lyme despite doing everything right. The right antimicrobials, the right detox, the right mindset. Yet they hit a wall. And more often than not, that hidden barrier was mold toxicity. Mold doesn't just cause sneezing or sinus congestion. It releases mycotoxins, potent immune disruptors that can suppress immune function, inflame the brain, and mimic or mask the very symptoms we often attribute to Lyme disease. Fatigue, brain fog, anxiety, body aches, gut issues. Mold can look just like Lyme. And without identifying it, patients may chase a wrong root cause for years. What's worse, when mold and Lyme collide, it creates a perfect storm. Mold weakens the immune system, which makes it harder to fight off infections like Borrelia, Bartonella, Babesia, and others. It's like trying to fight a war with no armor and low ammunition. So in today's episode, we're going to explore what mold toxicity really is and how it differs from simple mold exposure. The most common symptoms are mold illness and how they overlap with Lyme. Why mold must be addressed first in many complex cases before Lyme can be successfully treated. How to test for mold, what toxins to look for, and which treatment can actually make a difference. Whether you're a patient, practitioner, or just someone navigating unexplained chronic illness, this episode will give you practical tools and hope for moving forward. So let's dive in. Before we go any further, it's important to break down what mold illness actually is, because it's often misunderstood, even in the integrative health world. Let's start with this. Mold is not the whole problem. In fact, the real issue is what mold produces. Mold refers to the fungal organisms themselves. These are living, growing entities found in damp shape, humid environments. But mold doesn't need to be alive to hurt you. When under stress, certain species release mycotoxins, tiny, toxic chemicals that are airborne, invisible, and incredibly damaging. Think of it this mold is the factory, but mycotoxins are the chemical weapons it releases. These toxins are what makes people sick. Sick, and they're extremely hard to detect and even harder for the body to eliminate without help. And here's the kicker. Mycotoxins are fat soluble, which means they get stored in our fat tissue, our brain, and our nervous system, making them persistent and capable of weakening or wreaking havoc long after exposure. Now, there are two ways mold affects the body. Exposure and colonization.
Exposure is what most people are familiar with. It means you're breathing in mycotoxins from an environment like a water damaged home, workplace, or school. You don't have to see a visible mold for this to happen. Even hidden mold behind walls can release enough toxins to cause symptoms. But then there's colonization. And this is where things get tricky. Colonization means that mold has actually taken up residence inside your body. The sinuses, lungs, or gut can become home to fungal overgrowth, especially in people with compromised immune systems or chronic inflammation. Like many Lyme patients. In these cases, even after you've left a moldy environment, you still feel sick. Why? Because mold is now living inside you, producing mycotoxins internally. This is why some people never fully recover unless internal colonization is addressed with antifungals or specific protocols. So where does mold thrive? The most common sources are water damaged buildings, even a small leak behind drywall can create a breeding ground for mold. And then you have H vac systems, especially if not regularly cleaned. We have basements and attics, bathrooms without ventilation, old carpeting or furniture exposed to moisture, and even vehicles. I've seen cases where someone's car was the primary exposure. And remember, you don't need visible mold or a musty smell to have a mold problem. Many of the most dangerous species, like stachybotrys, asparagus, aspergillus, and penicillin, can be hidden and odorless.
Now that we understand what mold illness is and how it works, let's talk about how it shows up and why it's so often missed. One of the biggest challenges with mold illness is that its symptoms are wide ranging, non specific, and often identical to Lyme disease and other chronic conditions.
Let's break this down into three main categories. So we have the neurological symptoms. This is where mold hits hardest for many patients. We have brain fog. That is the most common symptoms we see. Patients describe it as feeling drugged, disconnected, or like their brain is wrapped in cotton. Then we have anxiety and panic attacks. These are often reported even in people who've never struggled with mental health before. Then we get depression and mood swings and even rage or suicidal. Ideation can be driven by neuroinflammation from mold toxins, especially gliotoxins and ochratoxins, which are known to affect neurotransmitter balance and mitochondrial function in the brain. And then we have the physical symptoms. So these mimic many of the physical complaints seen in Lyme, like crushing fatigue. No matter how much sleep a person gets, we get joint pain and body aches. We can easily be mistaken for Lyme flares or autoimmune issues. Then we get chronic sinus congestion or infections, sometimes misdiagnosed as allergies, but actually linked to fighting fungal overgrowth in the sinuses. And then we have the unusual signs. And then there are the most unusual symptoms that often get overlooked, like tinnitus. You know, we know it as ringing in the ears, which is surprisingly common in mold patients. And that can be such an incredible, incredibly difficult thing to get rid of. We have pelvic pain or interstitial cystitis, especially when exposure to zearlanone, a mold toxin that acts like estrogen and can inflame hormone sensitive tissues. Then we have sensitivities to smells, light or sound. These can indicate limbic system irritation.
One of the reasons mold illness is frequently misdiagnosed or never diagnosed at all is because it mimics other chronic illnesses, especially Lyme disease. And we already know Lyme as the great mimicker. Mold is very much the same. I can't tell you how many patients come into my clinic after Lyme diagnosis or co infections like Bartonella or Babesia that they're still not improving. They're chasing bugs. But what's actually blocking their progress is mold.
Both mold and Lyme can cause fatigue, brain fog, pain and neurological symptoms. Both can disrupt the immune system, leading to viral reactivations or worsening of co infections. And both can cause Herxheimer reactions when treatment starts, making it even harder to tell what's what. [00:12:25] Speaker B: Diagnosing mold toxicity isn't always straightforward, but we do have some tools. We got urine mycotoxin testing this is the most direct way to assess whether your body is burdened by mold toxins. Labs like Great Plains, Real Time or Vibrant can test for aflatoxins, okratoxins, gliotoxins, zearlanone, tricotecines and more. So keep in mind results can be influenced by binders, sweating or provoked detox, so working with a practitioner who understands how to interpret them is crucial.
Then we have another test. It's a visual Contrast Sensitivity Test VCS this is a non invasive quick screening tool that tests your ability to detect subtle changes in contrast. Mold toxins affect the optic nerve, so poor score can suggest neurotoxicity. You can take this test
[email protected] or similar platforms. Then other indicators like Marcon's nasal swab checks for antibiotic resistant staph in the sinuses common in mold affected patients. Then we have TGF Beta 1 MMP9C4A. These are inflammatory markers that may be elevated in mold illness. And then also a common test that we use is organic acid test or oats, and that can reveal fungal overgrowth or also mitochondrial dysfunction. It gives us a lot of information that we can act on. So the bottom line? If you or your patients are dealing with persistent Lyme symptoms and not responding to treatment, or if new mysterious symptoms have cropped up, it may be time to look beyond the bugs and into the building.
Now let's connect the dots between mold and Lyme and talk about why these conditions often show up together and more importantly, why mold must be addressed first. In many complex cases, mold toxins are stealthy saboteurs of the immune system. They don't just cause inflammation, they disrupt immune regulation. Here's how. So mycotoxins like gliotoxins can kill off immune cells, especially lymphocytes, making it harder for your body to detect and fight pathogens like Borrelia or Bartonella. Mold also suppresses macrophage activity, limiting your body's ability to clean up cellular debris or handle Herxheimer reactions. And it disrupts the gut barrier and microbiome, which is where 70 to 80% of your immune system resides. So if you're trying to treat Lyme but mold is still in the background, you're asking your immune system to run a marathon on a sprained ankle. Mold makes Lyme treatment less effective More reactive and more exhausting. This is why so many Lyme patients hit a wall. They plateau, flare up, or crash. It's not because the Lyme protocol isn't working. It's because the terrain is still toxic.
Both mold and Lyme trigger inflammation and trigger chronic inflammation. And together they create an internal wildfire that just keeps smoldering. So let's break it down. Lyme and co infections trigger the release of pro inflammatory cytokines like interleukin 6, TNF alpha and interleukin 1 beta. Mold toxins do the same and sometimes even upregulate TGF beta 1, a marker associated with tissue fibrosis and brain fog. So the result, you have a hyperreactive inflamed nervous system, a fatigued mitochondrial network, and a body stuck in fight or flight. Patients often feel wired but tired, overwhelmed by sound or light, emotionally fragile and physically drained. Their detox pathways shut down, their hormones crash, and their sleep becomes non restorative. You cannot put out a lime fire until you remove the mold soaked wood that keeps it burning.
So let me share a couple of recreated stories that just represents a number of my patients at my clinic that illustrate this mold Lyme interplay. So let's take sample case number one, the patient who failed Lyme treatment. So here we have a woman in her early 40s, came in with classic Lyme symptoms. Migratory joint pain, brain fog, insomnia, and crushing fatigue. She'd been on herbal antimicrobials, binders, and ozone therapy for over a year with only minor improvement.
Every time we pushed treatments, she crashed. So we revisited her story and discovered she lived in a beautifully remodeled home that had once flooded years before. We ran a urine mycotoxin panel and found elevated ochratoxins and zearlanone. Once she left the home and began targeted mold detox with glutathione, phosphatidylcholine and binders, her symptoms began to stabilize. Only then could we restart Lyme therapies. And this time, they worked. Within six months, her fatigue lifted and cognitive clarity returned. Then we have example case number two. The child misdiagnosed with anxiety. So another case involved a teenager labeled with anxiety, ADD and chronic fatigue. He'd been diagnosed with bartonella and treated, but symptoms kept rebounding. After further questioning, we learned the family had moved from a moldy apartment two years prior, but had kept most of the furniture. Urine testing revealed high levels of gliotoxins and trichotecines. Nasal swab confirmed Marcons colonization. So once we Implemented antifungal treatment, removed contaminated belongings, and introduced limbic system retraining. His symptoms improved dramatically. His focus, mood and energy began to normalize. So these stories are powerful reminders that Lyme and mold aren't just overlapping. They're often partners in crime. And if you don't treat the mold first, the body remains too inflamed, too toxic, and too weak to fight anything else.
So now that we've laid the groundwork, what mold is, how it presents, and how it intersects with Lyme, let's talk about the big question. How do we treat it? Here's the good news. Mold illness is treatable and patients can recover. But it requires a multi phase strategic approach. It's not about killing something like a bacteria. It's about removing toxins, restoring function, and rebuilding the terrain. Let's walk you through the key pillars of treatment that I use in my clinical practice. Step one is simple, but not always easy. You have to remove yourself from mold environments. Obviously, this becomes the biggest challenge if it's in your home or at work. So no supplements, no iv, no detox protocol will work if you're still breathing in the very toxins you're trying to get out. This means leaving the water damaged building, even temporarily running an ERMI or Hertzme 2 test to assess the home environment using HEPA air purifiers, mold safe cleaning products, and fogging tools like EC3 or BioBalance sprays from mold for mild exposures. I've seen patients spend thousands on treatments but stay sick because they didn't address the root source, their environment.
And sometimes it's not the house, it's the car, the office, or even their mattress or kept furniture from a moldy home. So whatever, please don't skip this step. Once the exposure stopped, or at least minimized, we begin detoxing with binders. These are substances that grab onto mold toxins in the gut and pull them out through the stool. Because mycotoxins are fat soluble and recirculate through the liver, if we don't bind them, they just keep getting reabsorbed. Some commonly used binders include cholestyramine. It is a prescription and especially good for ochratoxins and trichotokines. And then we have a prescription alternative, wellcol. We also have activated charcoal, a broad spectrum binder. It works well for many toxins. We got bentonite, clay and zeolite. These are excellent for aflatoxins and trichotechienes. Then we have chlorella. It can help with heavy Metals and also a number of different mycotoxins, humic and folic acids. They support detox and terrain repair. So the key is to take binders away from food supplements and medication, usually on an empty stomach, and drink plenty of water to keep bowels moving.
So in many patients, especially those with Lyme, we also have to deal with internal mold colonization. Mold doesn't just float around outside. It can take up residence in the sinuses, gut or lungs. For these cases we use anafungals, which may be nasal sprays like BG spray, amphotericin B or silver based formulas. We also get oral antifungals like itraconazole or fluconazole, or herbal blends containing oregano oil, berberine, thyme, olive leaf and pad arco.
One tip I often give if a patient has sinus congestion or chronic post nasal drip, we always test for marcons, which is multiple antibiotic resistant coagulase negative staph. This colonization must be cleared to restore the nasal biome and improve brain fog and fatigue. Clearing colonization often makes a dramatic difference in both energy and cognition. So when you're detoxing from mold, you're not just pulling out toxins, you're also repairing what those toxins damaged, calming the inflammation they caused and giving your detox organs the raw materials they need to function properly. So let's take a deeper look at some of these key supplements and why they matter. So we have glutathione. It is the cornerstone of detoxification, especially mold illness. Mold toxins like ochratoxins and trichotokines induce oxidative stress and impair mitochondrial function, particularly in the brain and liver. Glutathione does three critical things. It neutralizes free radicals created by mycotoxins. It supports Liver Phase 2 detoxification, helping package toxins for elimination. And it crosses into the brain, especially when we use as nebulizer IV forms to protect neurons from neuroinflammation, a key cause of brain fog and anxiety in mold patients. Because many mold patients are depleted in glutathione due to chronic inflammation, supplementing liposomal or IV form can make a dramatic difference in fatigue, cognitive clarity and resilience and herx reactions. And then we have nac. It is the building block for glutathione production, especially important for patients who can't tolerate direct glutathione or who need to support intracellular synthesis. But NAC is more than a precursor. Is mycolytic, helping to clear mucus from the lungs and sinuses? Especially important when mold colonizes these areas. It also supports liver detox and has been shown to protect against acetaminophen toxicity and other chemical insults, offering a layer of defense against mycotoxins. It also has a mild antiviral and anti inflammatory effect, helping reduce systemic load. Then phosphatidylcholine is a key phospholipid that helps rebuild damaged cellular membranes, crucial in mold illness, especially with ochratoxins, which directly damage phospholipid rich tissues like the brain, gut lining and mitochondria. But it also helps a second equally important role, stimulating bile flow. Since mold toxins are fat soluble and primarily excreted through the bile, Phosphatidylcholine helps mobilize and carry these toxins out of the liver and into the intestines. It also supports mitochondrial membrane health, improving energy and cellular resilience. Mold illness is a nutrient depleting storm, especially under the burden of detox and inflammation. So these three nutrient groups are critical. Magnesium, which is essential for over 300 enzymatic reactions including detox, stress regulation and sleep. It calms the nervous system and helps prevent detox related muscle cramps or heart palpitations. Vitamin C is a powerful antioxidant and antihistamine. It supports adrenal function, neutralizes oxidative stress from mycotoxins and helps regenerate glutathione iv. Vitamin C can also be used as a direct oxidative therapy for those who tolerate it well. Then B complex vitamins. Mold and chronic stress deplete B vitamins rapidly, especially B1 and B6. They're vital for nervous system health while B12 and we want to do methylated B12 and methylated folic acid to support methylation. Obviously we want to be cognizant if you are being over methylated. So that's there. It's really good to have a provider guide you. These are key to detox pathways. They also help manage fatigue, mood swings and poor concentration.
Mycotoxins trigger a cytokine storm, an overactive immune response that keeps the body in a state of alarm. This drives symptoms like body pain, brain fog and emotional liability. To calm the storm, we often use curcumin from turmeric. It's a powerful anti inflammatory that down regulates NF kappa beta, a master switch in chronic inflammation. Then we got resveratrol. It supports brain and mitochondrial health while acting as a natural mold toxin mitigator. And boswellia or frankincense. It's an AYURVEDIC herb with strong effects against joint pain, inflammation and even mast cell activation. These herbs help lower the volume on the immune response, allowing the detox pathways to work more smoothly. And let's not forget diet. Detox starch on your plate no mold protocol is complete without dietary change. Mold thrives in high sugar processed inflammatory terrain, so we shift the diet to support detox and reduce reactivity. Low sugar reduces fungal overgrowth and inflammation. High fiber helps bind toxins in the gut and promote daily elimination. Cruciferous vegetables like broccoli, kale and brussels sprouts stimulate phase one and two liver detox support hormone balance especially with serenone exposure and provide sulfur for glutathione synthesis. Then the anti inflammatory fats like avocados, olives, flax and fish oils help reduce cellular damage and support membrane health. So even without supplements, diet alone can begin to shift the terrain and unburden the liver. We also integrate therapies like infrared sauna, Epsom, salt bath, castor oil packs and lymphatic drainage to move toxins through the skin, gut and lymph. So one caution do not do it yourself without guidance. You know mold detox isn't something you want to jump into without a plan, especially if you're also treating Lyme. Why? Because detox pathways may be blocked, causing Herxheimer like reactions. Aggressive treatments can mobilize toxins faster than the body can eliminate them, leading to worsening fatigue, rashes, anxiety or even crashes. Combining antimicrobials with antifungals without supporting drainage can overwhelm the system. That's why we always start slow, test carefully and build up tolerance before layering therapies. Working with a practitioner understands both mold and Lyme is key because in complex chronic illness, order matters. And sometimes we have to pursue. We have to pass Lyme treatments until the body's strong enough to handle it.
One of the most misunderstood aspect of mold illness, and perhaps the most distressing for patients, is its impact on mental health. For many, it's not the fatigue or the joint pain that breaks them down. It's the brain fog, the emotional volatility, the sense that they're losing themselves. And sadly, many are told it's all in their head. But here's the truth. It is in the head, neurologically and biochemically. Because mold toxins can cause profound neuroinflammation. Mycotoxins like gliotoxins, ochratoxins, zearlanone can cross the blood, brain barrier, damage neurons and glial Cells disrupt neurotransmitters like dopamine, gaba, and serotonin. This can manifest like brain fog, memory issues, mood swings, anxiety, panic attacks, and depression, even intrusive thoughts or suicidal ideation. In severe cases, the brain becomes inflamed, overloaded, and unable to regulate thought and emotion clearly. And if the gut is compromised, which is often in mold patients, this further reduces serotonin production, impacting mental and emotional balance. Another layer is the limbic system, the brain's emotional control center, which includes the amygdala, hypothalamus, and hippocampus. In mold patients, the limbic system can get stuck in a constant state of threat detection. It becomes hypersensitive not just to mold, but to smells, foods, chemicals, sounds, and even people. We see this as chemical sensitivity, noise sensitivity, emotional reactivity, inability to calm down, even in safe environments. This isn't psychological, it's neurological. And until that overactive alarm system is rewired, healing often plateaus, even if the detox is going well.
So the therapeutic modalities to calm the brain becomes really important. This is where brain retraining comes in.
